The South London and Maudsley NHS Foundation Trust provides the widest range of NHS mental health services in the UK. We also provide substance misuse services for people who are addicted to drugs and alcohol. We work closely with the Institute of Psychiatry, Psychology and Neuroscience (IoPPN), King’s College London and are part of King’s Health Partners Academic Health Sciences Centre. We have about 4,800 staff working for us and serve a local population of 1.1 million people. We have more than 230 services including inpatient wards, outpatient and community services. We provide inpatient care for approximately 5,300 people each year and we treat more than 45,000 patients in the community in Lambeth, Southwark, Lewisham and Croydon. We are delighted to be part of the Lambeth Living Work Network Hub.
Thames Reach is a London-based charity which helps homeless and vulnerable people to live in decent homes, build supportive relationships and lead fulfilling lives. We also specialise in helping people with complex and multiple needs, including providing support to people with mental health issues and substance misuse problems. As part of the new expanded LWN Hub across the borough of Lambeth, provides support to people with mental health issues and social support needs. The service helps people in a broad range of areas including housing, benefits, education and employment and helps people address social isolation and increase their independence. It offers time limited, personally tailored support organised around what service users prioritise and choose. People are helped to become more resilient and to build new and strengthen existing social networks. The Thames Reach team also helps support service users to access personal health budgets which enable greater independence and a real choice about managing their own support. The short-term support is linked closely with the primary care services – the first point of call for people accessing health services such as GPs – and is less reliant on specialist secondary care such as community mental health Teams. It aims to give people more choice and a better access to a broader range of services. Staff work alongside primary care and clinical teams at the LWN Hub in Streatham and is made up of support staff from Thames Reach alongside those from other third sector organisations.

We listen to the people we support about how they want to spend their time and help them achieve it. We find out what people have to offer and help them develop it. This is a partnership and we are in this together. For people with learning disabilities or mental health needs – and for their carers and families – our support comes in many forms. From essential round the clock support to independent living in the community; from training and mentoring to build skills and confidence, to support in finding fulfilling and reasonably paid employment. And we provide our own opportunities through apprenticeships, work placement and volunteering roles. We encourage people to find new interests, make new friends, get out and about and have fun. For carers and families, we offer respite care and short breaks. We also work closely with local authorities, communities and others involved in delivering support. We recognise the barriers that stand in the way of the people we support and we are resourceful in our efforts to work with people to overcome them. We don’t work to a formula – we find new and better ways of doing things, we don’t judge and we never give up, even when the going gets tough. We see ourselves as partners and allies – working alongside people, carers and families. And ultimately, we measure our success on the happiness and richness of the lives people lead.
London Borough of Lambeth Adult Social Care Services provides its adult residents who have care and support needs associated with various physical and mental health difficulties with a wide range of care and support services as part of its statutory duties. Lambeth Adult Social Care’s core objectives are to promote wellbeing, prevent worsening of people’s care and support needs in the long-term, maximise choices and to improve participation of adult residents in the community. Lambeth Adult Social Care started working in partnership with the LWN Hub (Hub) in 2015 and has now firmly established itself within the Hub to deliver adult social care interventions to its adult residents with mental health difficulties. In line with the Care Act 2014, Lambeth Adult Social Care offers its adult residents and to their carers holistic assessment of their social care and support needs as part of Hub’s routine services. A wide range of social care interventions are delivered to adults in collaboration with the other partner organisations within the Hub to ensure prevention of worsening of needs. As part of its commitment to safeguard vulnerable adults with mental health difficulties from abuse and exploitation, Lambeth Adult Social Care has been carrying out its statutory adult safeguarding duties as part of the Hub’s core functions. Additionally, provision of information, advice and services in collaboration with other social and health partners forms an essential part of Lambeth Adult Social Care’s presence in the Hub to promote wellbeing and prevent long-term needs.
