The Lambeth Living Well Network Hub receives funding through Guy’s and St Thomas’ Charity. Part of the funding was to undertake an evaluation programme to assess the impact and develop the evidence based for community based provision.

The report shows that the Living Well Network Hub is having a positive impact in Lambeth.  The Hub has increased access to mental health support in primary care, has contributed to a reduction in the number of referrals to the Assessment and Liaison service (A&L) by 31%, referrals to secondary care by 25% and a reduction of caseloads oo long term care co-ordination by 27%.
